เมื่อ 4/6/2548 7:15:10		ผู้ป่วยหญิงอายุ 28 ปี No Underlying disease มาด้วยมีอาการเหนื่อยง่ายมา 1 เดือน FC 1 -->3 เดินขึ้นบันไดได้ 2 ชั้นก็เหนื่อยแล้ว ไม่มี PND / ไม่มี Orthopnea ไม่มีใจสั่น ไม่มีเหงื่อออกง่าย ไม่มีไข้ มีน้ำหนักลดลงมากแต่ไม่ทราบว่ากี่กิโล ปัสสาวะ อุจจาระปกติ

เมื่อ 4/6/2548 7:20:47		มาตรวจ OPD วันนี้เพราะต้องการรู้ว่าทำไมน้ำหนักลดมาก ไม่ได้ concern กับเรื่องเหนื่อยมาก ช่วง 1 เดือนที่ผ่านมาสังเกตว่าขาบวม 2 ข้าง เพิ่งมีคนทักว่าตาเหลืองประมาณ 1 อาทิตย์ก่อน ไม่มีปวดจุกท้อง ประจำเดือนมาปกติ ประวัติอดีต แข็งแรงดีมาตลอด social alcohol drinking ไม่สูบบุหรี่ Negative Family History

เมื่อ 4/6/2548 7:24:43		case นี้ส่งตัวมาจาก clinic อีกที มี order ให้ตรวจ CXR กับ EKG ก่อนพบแพทย์ พอ nurse aids เอา EKG มาให้ดู ระหว่างคนไข้รอ CXR อยู่ เรารีบวิ่งให้ส่งคนไข้ไป ER เดี๋ยวนี้

EKG เป็น AF, rate 180/min , LVH , No ST -T change

Good conscious , Marked cachexia , Not pale , Marked Icteric sclera
Thyroid gland 25 g , no bruit , no exopthalmos , no tremor
Chest : clear JVP 5 cm H2O
Heart : Heave at apex , no thrill , tachycardia , normal S1 S2 , murmur can't be heard
Abdomen : Soft , Liver and Spleen can't be palpated
Pitting edema 2+ both legs
Skin : No signs of chronic liver stigmata

เมื่อ 4/6/2548 7:36:21		 พอเห็นตอนแรกก็คิดถึง CHF ก่อน แต่ไม่รู้ว่าทำไมเหลือง ประวัติเรื่องยามีแต่เมื่อ 2 อาทิตย์ทานยาที่ได้มาจากญาติที่อเมริกาให้มาเพื่อบำรุงร่างกาย เอามาดูก็เป็นแค่ Folic acid ไม่มีประวัติใช้ยาสมุนไพร ถึงตอนนี้ก็เลยพยายาม slow HR ก่อน ให้ digoxin 0.25 mg IV ผ่านไป 20 นาที HR 180 --> 150 ทำ CXR ได้ผลดังรูป

Problem List
1. Dyspnea on exertion : Pitting edema
2. Marked jaundice
3. Atrial fibrillation with Rapid ventricular response
4. cachexia

ในทั้งหมดนี้..คงต้อง approach ทีละเรื่อง
เรื่อง อาการเหนื่อย เนี่ย... ประวัติสั้นไปนิด...แต่ไม่ได้ suggest ว่าเป็น Heart dz / Lung dz
อันนี้..สงสัยไม่ได้กะให้ discuss ที่ประวัติ..เนอะ... เพราะ PE + CxR มี Pitting edema กับ Cardiomegaly ...คงจะเหนื่อยจาก Heart ล่ะ...ข้อค้านมีอยู่บ้างตรงที่
PE ไม่มี Crepitation อันนี้ถ้าจะว่าเหนื่อยจาก Heart failure คงจะแปลกไปบ้าง... แต่ พอคิดรวมกับ Pitting edema แสดงถึง Right sided Heart failure / Liver disease / Renal disease ... ทำให้นึกถึง Heart มากขึ้น... เพราะ ถ้าเป็น Renal disease คนไข้ก็ตัวผอมมาก ๆ ไม่เหมือน ทั้ง Nephrotic / renal failure ... ถ้าเป็น Liver disease ก็ไม่พบว่า PE มี Portal hypertension อื่น ๆ ... ไม่มี ascites superficial vein dilate spleenomegaly
ก็มารวมกันที่ Heart disease อีก...

เมื่อ 4/6/2548 16:30:50		ทีนี้...ไอ้ Right sided heart failure กับเรื่อง เหนื่อย ..(ซึ่งจริง ๆ อาจจะเป็นแค่ใจสั่นแล้วเลยเหมือนว่าเหนื่อย) ... อันแสดงถึง Heart dz เนี่ย... ต้องดูว่าเป็น Structural heart dz หรือ Conduction problem ซึ่ง Structural ก็เป็นได้ตั้งแต่ Endocardium / Myoccardium / Pericardium ... ขึ้นกับตรวจร่างกาย กับ ประวัติละ... ส่วน Conduction problem ...คนนี้มีแน่ ๆ ... เพราะเป็น Af RVR อยู่ ... แต่ครับ...แต่ว่า Af RVR กับ Heart failure เนี่ยต้องระวังให้ดี..เพราะเราจะไม่รู้ว่าอะไรเกิดก่อนกัน...เค้าอาจจะมี Af อยุ่ก่อน แล้ว เกิด Heart failure ...หรือ เป็น Heart failure precipitate เกิด Af RVR ก็ได้... เอาเหอะ..
เรื่องของ Stuctural heart ...คนนี้ Pericardium ...ตรวจร่างกายไม่มี Faint heart sound รึเปล่า ? .. EKG ไม่มี Low voltage ..
Myocardium .. อายุน้อย...ไม่มี Risk ...เป็น ได้ยาก.. ก็มี Myocarditis เนี่ย..พอไหว...อธิบายทุกอย่างพอได้...แต่..ไม่อธิบายเรื่อง เหลือง ..
Endocardium อันนี้..ต้องฟัง Heart .. ถ้า Rate ขนาดนี้..หมดสิทธิ์.. แต่ดูจาก ประวัติ..เป็นเร็วแบบนี้..มันจะไม่เหมือน ... กว่า MS ซึ่งจะอธิบาย Left sided กับ Af ได้..จะทำให้มี ขาบวม ..มันน่าจะมี PND orthopnea มาก่อน.. จะอธิบายได้ก็คือ Rheumatic heart disease ... ซึ่งมี heart โต ๆ ..มี sign left sided / Right sided ปนกันได้.. ต้องถามประวัติ ดี ๆ... คนนี้ไม่มีไข้มาก่อนใช่ป่าว...

สรุปว่า..เหนื่อย บวม มาจาก heart นั่นล่ะ... แต่จะ precipatate จากอะไรเนี่ยสิ...น่าคิด

1. เหลืองมาทีหลัง heart failure ... อาจจะเป็น complication ของ heart failure เช่น..Chronic heart failure มี Cardiac cirrhosis ..ซึ่งคนนี้มีแต่เหลือง ..ไม่มี sign Cirrhosis
2...มี common etiology ร่วมกัน... เพียงแต่ เหลืองมาทีหลัง
อันนี้..จะตกอยู่ในกลุ่ม ...Systemic cause ..อันได้แก่...
2.1 .. Systemic infection
2.2 .. Endocrine disease
2.3 .. Drug induced / toxin
2.4 .. Autoimmune
ถ้าไม่มี Hx ไข้เลย...คงไม่นึกถึง infection นะ..
Drug induced ... ลองถามประวัติดู... Steriod / NSAIDs ... ทั้งคู่มี Hepatotoxic + Salt water retention .. อธิบายทั้งคู่ได้อยู่... แต่ cachexia นี่ล่ะ... เป็น PU จนกินไม่ได้เหรอ? ฮ่ะ ๆ ... ไม่แน่ใจ
Autoimmune ...อันนี้น่าสนใจ ..ต้องไปตรวจร่างกายดูหา evidence อื่น ๆ ..
Endocrine ... อันนี้แหละ...ที่เราจะบอก...

เราว่าเป็น Thyroid strom ... แหละ...สรุปว่างั้นละกัน...

อธิบายได้ทั้ง Heart failure / CAchexia / Af RVR / Jaundice

ต้องไปคิด Metzinsky score ดู... ว่า Diag ได้มั้ย...

วันนี้ตอบเฉพาะประวัติที่ถามเพื่มแล้วกัน ช่วง 1 เดือนมานี้มีเหนื่อยขึ้น ขาเพิ่งมาบวมช่วงหลัง ไม่มีอาการเจ็บหน้าอกหรือใจสั่นเลย < ขนาดอยู่ใน ER HR 180/min ก็บอกว่าปกติดี จะขอกลับบ้านด้วย > ยืนยันว่าไม่มีไข้ ไม่มีปวดจุกท้อง ปัสสาวะ อุจจาระปกติ เค้นประวัติเรื่องยาก็มีแต่ Folic acid ที่เอามาจากอเมริกา เรื่องเหลืองเพิ่งมีคนมาทัก 1 อาทิตย์ก่อน < คนไข้ตอบแบบ ไม่ค่อยสนใจตัวเองเท่าไร เหลืองขนาดนี้ยังไม่รู้ตัวเลย >

ตรวจร่างกาย HR เร็วมาก แต่ได้ยินชัดดี

มีใคร D/DX อะไรอีกบ้างไหม และจะส่ง investigation อะไรต่อ initial treatment หลังให้ digoxin มีอะไรอีกบ้าง

เรื่อง ... Af rapid ventricular response ...คงต้องระวังว่าจะเป็น การส่งต่อ impulse จาก atrial ไป ventricle ผ่าน accessory pathway ...เพราะลงทาง AV node มักจะถูก delayed ไป...ซึ่ง associate กับ Endocardial cushion defect ..
Investigation อันที่จะ specific คงเป็น Thyroid function test ..ครับ..
ส่วน cardiac enzyme อาจจะส่งไป R/O structural myocardial disease

เมื่อวานไปนั่งคิดแล้ว... systemic ที่มาอาการ non specific ยังมี Malignancy อีกกลุ่มนะ... แต่คงไม่อธิบาย Heart failure มั้ง

ตอนแรกที่เจอ case นี้ก็อธิบายเรื่องเหลืองไม่ได้เหมือนกัน รู้แต่ว่ามี AF ในคนอายุน้อย ถ้าไม่เป็น structural heart ก็ hyperthyroid เลยส่ง FT3 กับ TSH ไปด้วย หลังได้ digoxin ก็ยังเร็วอยู่ เลย admit ICU ให้ digoxin dose ที่ 2 HR ก็ยังอยู่ช่วง 120 อยู่ ไม่ได้ให้ diuretic เพราะยังไม่แน่ใจเรื่อง CHF เพราะไม่มี signs ของ acute pulmonary edema เลย ถ้าเป็นก็น่าจะเป็น Rt side มากกว่า high output CHF แต่ก็แย้งตรงที่ signs Rt side CHF ก็ไม่ค่อยพบ < JVP ประเมินยากเพราะคนไข้ผอมมาก และที่บรรยายก็เป็น external jugular vein > พอ lab กลับมายิ่งงงว่า case นี้มี CHF หรือเปล่า

CBC : Hct 38 % WBC 10,200 < N55 % L 30%> Plt 120,000
BUN 10 Cr 0.5 E'lyte : WNL
LFT : Albumin 2.0 Globulin 3.5 SGOT 45 SGPT 36 ALP 85 TB 20 DB 12

TFT : Pending


ถามต่อ -->

- คนนี้จัดว่ามี CHF หรือไม่ ต้องให้ diuretic หรือไม่ < CXR ค่อนข้าง clear / เรื่องบวมก็อธิบายได้จาก hypoalbuminemia >
- ตอนนี้ได้ digoxin ไป 2 dose แล้วก็ยังไม่ลง ทำยังไงต่อดี
- TFT case นี้เราสั่ง FT3 กับ TSH การแปลผลต้องระวังอย่างไร เพื่อไม่ให้สับสนกับ Euthyroid sick syndrome ในคนไข้ที่ severe medical illness
- เรื่อง Jaundice จะ work up อะไรต่อ จำเป็นต้อง request U/S Emergency หรือไม่ < ถ้าไม่ส่ง U/S จะผิดหรือเปล่า >

พี่ admin จะร่วม discuss เรื่อง cardio ก็ได้น่ะครับ ผมก็ยังงง ๆ อยู่

เมื่อ 7/6/2548 21:25:54		To support giggug, it could be possible when a thyrotoxic crisis patient becomes dyspnoea without left sided hf. When the patient has undergone long standing hyperthyroidism, in some reports, pulmonary hypertension can develop. Thus it reduces blood flow to the lungs and eventually, decreases co and there come dyspnoeic symptoms. However, the strange history is the patient has just experienced the dz for 1 months. In classic case of the condition mentioned earlier, the dz has got to appear long enough prior to the vascular maladaption. Nevertheless, asymptomatic hyperthyroid may gradually subtle and insidious while the condition develops simultaneously. Any suggestions?

Something im gonna ask.Concerning bursh's thyroid crisis score, im not sure whether the jaundice can be explaint by any reasons prior to categorizing it as idiopathic hyperbilirubinemia. So, may I request blood smear and LFT in order to sort out the explainable source of the finding.

I don't know.I only know Wartofsky 's score from Professor Burch HB and Professor Wartofsky Leonard from Department of Medicine, Walter Reed Army Medical Center, Washington, DC.


They constructed a diagnostic point scale for the purpose of enabling a semiquantitative distinction between uncomplicated thyrotoxicosis / impending thyroid storm / established thyrotoxic storm.They analysed 61 cases of severe thyrotoxicosis in the literature since 1926.


You should read their original review article about this entitiy.This paper was cited as reference more than 10 year ago.


Burch, HB, Wartofsky, L. Life-threatening thyrotoxicosis. Thyroid storm. Endocrinol Metab Clin North Am 1993; 22:263.

For this case,I calculated from AF/Unexplained Jaundice/Tachycardia = 55 pts.A score of 45 or greater is highly suggestive of thyroid storm.Unexplained Jaundice is recognized since 1927 and is emphasized again in 1945.It was postulated from marked hepatocellular dysfunction.

Last but not least ...

Thyroid storm is a dreaded,fortunately rare complication of a very common disorder.Effective management is predicated on a prompt recognition of impending thyroid storm which is dependent on a through knwledge of both the typical and atypical presentations of this order.It had been reported as multiple organ failure / acute hepatic failure / stroke / coma / acute abdomen or even cause of complete heart block.

Remember that this entity must use clinical diagnosis , not laboratory diagnosis.In this year,I found 3 cases with this entity.

ฟังที่ HR เท่าไหร่ครับ
AF ไม่ respond ต่อ digoxin มันคงต้องหาสาเหตุกันว่ามี ongoing stress อะไรหรือเปล่า ให้ซ้ำก็พอไหวอยู่นะครับถ้าไม่เคยได้มาก่อน เห็นcardiac shadow อย่างงั้นผมไม่อยากให้ยา antiarrhythmic อื่น
เอา HR ลง ทำ echo เพราะ CXR มัน mitral configuration เหลือเกิน ผมอยาก R/O MS อยู่ดีถึง film ไม่มีลักษณะ pulmonary congestion ที่ชัดเจน ส่วน LFT ผมงงเหมือนกัน
นอกเรื่องนิดนึงครับ

เมื่อ 8/6/2548 15:53:15		ตรวจร่างกายตอนแรกฟัง HR ที่ 180 พอ slow ลงเหลือ 120 ก็ยังฟังไม่ได้ยิน

สรุปพี่ยังคิดว่า case นี้เป็น CHF หรือครับ ในช่วงแรกจะ treat โดยให้ diuretic/ACEI ไปเลยหรือเปล่าครับ

คนไข้มา ER ตอนเที่ยง แรกสุดก็ D/DX Thyroid Storm ไว้เหมือนกัน แต่ติดใจตรงที่คอไม่โต และไม่มี tremor ร่วมกับยังคิดเรื่อง Jaundice ไม่ออกว่าเป็นหนึ่งใน criteria ด้วย และ supportive treatment ไปก่อน plan จะ admit ICU แต่เต็ม เลยต้องไว้ ward ธรรมดาแล้วติด tele monitor EKG

Social History

case นี้น่าสงสารมาก ตอนมาตรวจครั้งแรก พาลูกสาว 2 คนมาอายุ 5 ขวบกับ 6 ขวบมา กะว่าตรวจไม่นานแล้วจะเอาลูกไปโรงเรียนวันแรก ขนาดบอกว่าจะหา ICU ให้นอน คนไข้ก็จะขอกลับไปส่งลูกก่อนแล้วจะมาอีกทีตอนบ่าย แต่เราก็ไม่ยอม ปัจจุบันคนไข้อยู่กับแม่อายุ 80 ปี หย่ากับสามีไป 3 ปีแล้ว ต้องเลี้ยงลูกคนเดียว 2 คน ทำงานรับจ้างรายวัน ลูก 2 คนก็วิ่งเล่นไปมา ยังไม่รู้ว่าแม่ตัวเองอาการหนักแค่ไหน

เมื่อ 9/6/2548 12:03:40		 หลังจากนั้น HR ก็ยังอยู่ระดับ 120 BP Stable ดี พอ admit ไปได้ 2 ชั่วโมงเริ่มมีไข้ต่ำ ๆ < BT 37.5 > ก็ทำ septic workup ไป แต่ก็ยังไม่กล้า treat thyroid storm ไปเลย ตอน 6 โมงเย็นพยาบาลโทรมาบอกว่าผล thyroid ออกแล้ว < ไม่น่าเชื่อ ออกเร็วมาก > FT3 120 pg/ml (1.6-4.5) TSH < 0.01 (0.3-4.1) เลยมั่นใจว่าเจอ case thyroid storm แน่ ๆ ที่มาด้วย acute hepatic failure
ICU ก็ยังเต็มอยู่ ตอนนั้น 6 โมงกว่าแล้ว เราไม่ได้อยู่เวร แต่ก็ order ทางโทรศัพท์ ให้หา ICU เข้าให้ได้ เพราะเป็น emergency แล้วให้ cocktail therapy ไป

PTU <50 mg> 4 tab q 4 hr ( 1,200 mg/day)
Dexa 2 mg IV q 6 hr
Lugol 's solution 10 drop OD ( plan ให้หลัง start PTU ไปแล้ว 2 ชั่วโมง )
Lasix 40 mg IV
Atenolol (100) 1 tab OD
Thiamine 100 mg IV OD
Vitamin K 10 mg IV OD X 3 days


อีก 2 วัน ถัดมาได้กลับไปอยู่เวรอีกเลยตามไปดู ก็ยังหา ICU เข้าไม่ได้ แต่คนไข้ดูดีขึ้นมาก ไม่มีไข้อีก HR 100 /min <AF> ยังเหลืองเท่าเดิม แต่ยื้มพูดคุยได้ดีมาก เราก็ force ให้หา ICU เข้าให้ได้ ผล U/S Upper abdomen ปกติ ตอนแรกยังอยากจะให้ cordarone คุม rate ให้ต่ำกว่านี้ แต่เราไม่ได้เป็นเจ้าของ case เลยทำอะไรไม่ได้

เมื่อ 9/6/2548 12:21:55		 ล่าสุดเมื่อวาน < ผ่านไป 1 อาทิตย์ > ได้ไปอยู่เวรอีกรอบ หยิบกล้องเอาไปกะจะไปถ่ายเปรียบเทียบว่าตาหายเหลืองหรือยัง พอเดินไปที่ ward บอกว่าคนไข้เมื่อ 4 วันก่อน เริ่มซึมลงแต่ยังทำตามสั่งได้ ICU ว่างเลยย้ายไป observe ได้ 2 วัน ไม่มีไข้ HR ก็ยังเป็น AF อยู่ rate 120 /min แพทย์เจ้าของไข้คุย prognosis กับญาติว่า grave prognosis แม่อายุ 80 ปี เลยทำใจ sign NR แต่คนไข้ยังรู้ตัวอยู่ แต่ไม่มีแรงลุกจากเตียง ล่าสุดเมื่อวานมี case ที่ต้องการใข้ ICU เลยย้ายคนไข้กลับไป ward ธรรมดา ในสภาพที่ยังซึมอยู่ จนเมื่อวานตอนเช้า BP เริ่ม drop ลง แล้วก็ death ไป

case นี้เป็น case ที่น่าเสียดายมาก ๆๆๆๆๆๆๆ ตอนพยาบาลบอกว่าคนไม่อยู่ ward ธรรมดาแล้ว ยังคิดว่าคงกลับบ้านไปแล้ว เลยอดถ่ายรูปมาให้ดูเลย

Final Diagnosis : Thyroid Storm with Acute hepatic faitlure with Multiple Oragans Failure
Final Result : Death

ขอบคุณคนไข้รายนี้ ที่อนุญาตให้ถ่ายรูปมาให้พวกเรา discuss กันได้ และเป็นรูปสุดท้ายในชีวิตของเค้าด้วย น่าสงสารลูก 2 คนจะอยู่ยังไงต่อ ถ้า case นี้อยู่ในโรงเรียนแพทย์อาจพอมีโอกาสรอดได้บ้าง ถ้า aggressive treatment มากกว่านี้ salvage treatment ของ thyroid storm ที่เคยรายงานแล้วรอด ได้แก่ plasmapheresis / Lithium high dose / thyroidectomy / radiocontrast organic iodide

เมื่อ 10/6/2548 16:54:20		ไม่ต้องเสียใจหรอกนะ มัน Grave Prognosis อยู่แล้วน่ะ ถ้าเราทำเต็มที่แล้วก็ OK เราก็เคยเจอ แย่กว่านี้อีกตรงที่คนไข้มาวันเสาร์ Lab บอกว่าตรวจ TFT ไม่ได้ ขนาดเราบอกว่าด่วนมาก ก็ไม่ทำให้ ต้องรอวันจันทร์ เราก็เลยรักษา HF +AF ไปก่อน พอตอนกลางคืน มี ชัก GTC เราเลยโคตรมั่นใจ Rx as thyroid strom เลย แต่ไม่นานคนไข้ก็ Death (คนไข้เราไม่มีเหลืองนะ มาด้วย AF+HF inyoung adult + lid lag )

ที่เราบ่นว่าเสียดาย ก็เพราะ case นี้มันไม่เต็มที่เลย < มีเบื้องหลังอยู่เยอะ > case นี้เราพลาดตรงที่ไม่กล้า treat thyroid storm ไปเลย เพราะมันเป็น clinical diag ผล lab ไม่สัมพันธ์กับ clinical ถ้าแค่สงสัย หรือเป็น case ที่มีประวัติ euthyroid แล้วมาเจอ stress หนัก ๆ เช่น trauma หรือ sepsis ก็ให้ treat as thyroid storm ไปแต่แรก ข้อเสียอย่างมากก็มี idiosyncrasy side effect ของ PTU แต่ไม่ early diag แล้ว sequele ตามมามากกว่านั้นมาก

case นี้ที่เราบ่นเยอะ เพราะคนไข้ดูดีมากตอนแรก และก็ treat ค่อนข้างเร็ว <แม้จะ delay ไปครึ่งวัน > วันรุ่งขึ้นก็ดีขึ้นมาก แต่พอเริ่มมี storm เต็มรูปแบบ กลับไม่ aggressive treatment ไป sign NR ใน case ที่ควรสู้เต็มที่ ET tube ยังไม่ได้ใส่เลย
