I don't know.I only know Wartofsky 's score from Professor Burch HB and Professor Wartofsky Leonard from Department of Medicine, Walter Reed Army Medical Center, Washington, DC.
They constructed a diagnostic point scale for the purpose of enabling a semiquantitative distinction between uncomplicated thyrotoxicosis / impending thyroid storm / established thyrotoxic storm.They analysed 61 cases of severe thyrotoxicosis in the literature since 1926.

For this case,I calculated from AF/Unexplained Jaundice/Tachycardia = 55 pts.A score of 45 or greater is highly suggestive of thyroid storm.Unexplained Jaundice is recognized since 1927 and is emphasized again in 1945.It was postulated from marked hepatocellular dysfunction.

Thyroid storm is a dreaded,fortunately rare complication of a very common disorder.Effective management is predicated on a prompt recognition of impending thyroid storm which is dependent on a through knwledge of both the typical and atypical presentations of this order.It had been reported as multiple organ failure / acute hepatic failure / stroke / coma / acute abdomen or even cause of complete heart block.
Remember that this entity must use clinical diagnosis , not laboratory diagnosis.In this year,I found 3 cases with this entity. |
